Transition Services provides supports and additional funding to assist eligible participants to return to or remain in the community.

Transition Navigators work one-on-one with participants to plan
and facilitate the transition process. Our Transition Navigators take great care to thoroughly assess the person’s unique needs and circumstances in order to be successful. Their deep experience also helps identify risk factors that may jeopardize a successful transition to the community— an overlooked small thing, such as a well-placed grab bar, can make the difference between success and a fall.
